ST. JOSEPH HEALTH CARE TRUST
Due to the ever rising costs of health care, more people in our community are medically uninsured or underinsured, making it difficult for them to access primary medical care. This causes many individuals to wait to access medical services until it is an emergency. Emergency care is the most costly care. The St. Joseph Health Care Trust strives to connect the uninsured and underinsured to health care services at the primary care level in order to detect and treat illnesses at the earliest stages. Our Health Systems Navigators assist clients with connecting to primary medical care providers to diagnose and treat illness at its earliest stages. Health Systems Navigators also connect clients to prevention services and health education services to help prevent disease and provide information that can lead to living a healthier lifestyle.

To promote a healthier community by increasing access to medical care and increasing opportunities for health education.
St. Joseph Health Care Trust partners with parishes and community-based organizations to offer medical services to low income, uninsured or under insured individuals and families. Services include:
- Direct medical assistance
- Vision services
- Dental services
- Counseling (through approved providers)
- Durable medical goods
- Preventative health care
- Health screenings
- Health fairs
- Pharmaceutical assistance
- Health education
- Health systems navigation
Spanish-speaking staff members are available. Other language needs can be accommodated.

Service Criteria:
This program offers services to uninsured and under insured individuals and families residing in one of the 28 counties of the Diocese of Fort Worth whose income falls at or below 200% of the Federal Poverty Guidelines.
Cost:
Clients are asked to financially contribute toward the cost of services. A minimum copayment of $10 is required. The copayment may be higher depending on the type of service and client income. Fees for dental services are based on the type of procedure.
